ACKNOWLEDGEMENT OF RISKS & RESPONSIBILITY


I, the undersigned, acknowledge, agree, and confirm the following:

  1. Voluntary Participation:
    I am voluntarily participating in activities provided by Aweh Be Lekker, including but not limited to jetboarding, paddle boarding, surfing, equipment rentals, and other outdoor/adventure experiences.

  2. Risks Involved:
    I understand that these activities may involve risks, including but not limited to: injury, drowning, collisions, equipment malfunction, slips or falls, natural hazards, or unforeseen circumstances. I accept full responsibility for my safety and the safety of minors under my care.

  3. Health & Fitness:
    I certify that I am physically fit, in good health, and able to safely participate in the chosen activities. I will notify the Aweh Be Lekker staff of any medical conditions, allergies, or concerns prior to participation.

  4. Equipment Usage:
    I agree to use all equipment responsibly and as instructed. I acknowledge that I will be held liable for any damage or loss caused to equipment due to misuse or negligence.

  5. Compliance with Rules:
    I agree to adhere to all safety instructions and guidelines provided by Aweh Be Lekker staff. I will follow all rules to minimize risks during activities.

  6. Assumption of Risk & Release of Liability:
    I assume all risks associated with the activities and hereby release, discharge, and indemnify Aweh Be Lekker (including its owners, staff, instructors, affiliates, venues, locations, and partners) from any and all claims, damages, losses, injuries, or liabilities, whether caused by negligence or otherwise.

  7. Venue/Location Liability:
    I acknowledge that any venues or locations used for activities are cleared of all liability. I agree not to hold any property owners, managers, or venue operators liable for injuries, damages, or losses sustained during activities.

  8. Media Release:
    I consent to the use of photos or videos taken during activities for promotional purposes by Aweh Be Lekker, unless I specifically opt out in writing.

  9. Parental/Guardian Consent:
    (If participant is under 18 years old): I, as the parent/guardian, give permission for the above-mentioned minor to participate in activities and agree to all terms outlined in this indemnity form.
